Acn: agreement signed, now let's get ready for the change convention. Scotti: today's signature brings together the past and the future

The agreement between SISAC and general medicine unions was signed today. The answer to the "past" expectations of professionals who had been economically stopped for 8 years is coming, i.e. the economic arrears for all sectors: family doctors, continuity of care, territorial health emergencies and territorial services. But the first important answers for the "future" of general medicine also arrive on the legislation:

speeding up access to the profession of young doctors and guidelines for regional agreements which aim to increase the intensity of care through first-level diagnostics in studies, recognition of the central role of the family doctor in the management of the chronic patient and the one active in vaccinations and participation in the government of waiting lists.

"It is a starting point of great value for the declared commitment to support the evolution of general medicine which, in respect and continuity of its tradition and its peculiarities, is recognized as a reference subject for responding to the health requests of Italian citizens guaranteeing the sustainability of the NHS". This is the comment of the national secretary of the FIMMG Silvestro Scotti. “It was a team game between the ministry, the regions and the union – continues Scotti – which made it possible to provide special protections for pregnant female colleagues and for young people who acquire specific training qualifications. It has also been envisaged that continuity of care activities are carried out in suitable locations to ensure the safety of colleagues in particular, thus beginning to keep a promise we made at our last Congress.

The large trade union sharing then represents an added value if it is possible to maintain unity of purpose on decentralized agreements despite the diversity.

"It's not just good news - adds Scotti - since what had seemed solvable in a short time, i.e. an increase in the number of scholarships through the use of plan funds finalized by the Ministry of Health for this purpose, seems to have been blocked due to opposition from only one region. We remind this Region that speeding up access without training new doctors will make the situation worse, making the emergency a fact of its exclusive responsibility. We will be firm as regards the continuation of the negotiations, above all on the subject of prison assistance which we cannot load on the numbers of the diploma in General Medicine such as those of the current programming, so whoever takes these decisions is perhaps underestimating the problem which, in the case of prisoners, could even become dramatic.

However, we still want to be optimistic, also because we are in a moment of new adjustment of the national political landscape, - concludes Scotti - and once the right measures are found, we will continue the dialogue with the Regions and with the government structure that will be set up for a process of a renewal which however always maintains the same commitment and the same gaze aimed at the citizen's interest and the constitutional defense of his health”.

ACN 2018: Security, Access and Maternity.

FIMMG Continuity of Care expresses satisfaction for the signature of the ACN which achieves objectives on which it has been working for years, starting from the declaration in the minutes on the safety of the Doctors of Continuity of Care.

In fact, the Local Health Authorities are committed to identifying only suitable and safe locations for outpatient activities with free access for citizens.

The Regions also undertake to define the opening hours of the clinics only during the day and in the evening, considering that the 70% of assaults takes place at night, in any case keeping the service active during the night with telephone counseling and home visits.

This is a first concrete and effective step in dealing with the safety emergency which we will monitor with extreme commitment so that the managers of the Healthcare Companies implement the provisions of the ACN in the shortest possible time throughout the country.

Finally, the long-awaited solutions to the problems that women doctors of Continuity of Care, i.e. the most represented group of doctors in the sector, have to face in the event of maternity have been achieved: from the possibility of being included in the regional rankings as soon as they have obtained their general medicine title, absorbing the current delay of a year and a half, to safeguard the score for the regional ranking in the event of suspension for maternity leave.

Measures that we do not consider exhaustive, but a first sign of attention and a first milestone achieved for an increasingly female category.

The Continuity of Care system is strengthened, thanks to the new possibilities for acquiring permanent positions and the priority for replacements for those who attend the three-year period of specific training; in fact, those who have invested in general medicine are valued, thus stabilizing the sector thanks to doctors who, having made a precise vocational choice, will move more easily and quickly from temporary positions to those of full ownership.

Today's signature is an important one: a signature that we consider the beginning of a journey that lays the foundations for a contractual renewal in which the great challenges of chronicity, prevention and initiative medicine will involve, equally, all the Doctors of General medicine.

PINA ONOTRI, SMI: “INACCEPTABLE AND CONFUSED SISAC POSITIONS ON: ROLE OF DOCTORS IN NATIONAL HEALTH PLANS, SUCH AS CHRONICITY; ON THE RIGHT TO STRIKE AND THE CRITERIA FOR ASSIGNING TEMPORARY POSITIONS"

March 29 - The planned meeting between the medical unions and the public counterpart, Sisac, was held today.

Present for the SMI were the general secretary, Pina Onotri, the national leaders, Enzo Scafuro (resp. Convenzionate), Alfredo Granito (continuity of care), Nicola Marini (118), Floriana Riddei (medicine of services).

At the end of the meeting, the general secretary, Pina Onotri, contested the text presented by Sisac, because "behind the screen of a due act, the recognition of arrears, important regulatory aspects change".

“Meanwhile – explains Onotri – the role of doctors is defined in a punitive and confused sense, with respect to national plans on chronicity, vaccines, and the government of waiting lists. Unacceptable.

Therefore, the criteria for assigning provisional assignments are not correctly specified.

Finally, the right to strike is called into question by weakening it”.

"Like SMI we will convene the national council on these issues - concludes Onotri - only after that we will evaluate whether to affix a technical signature, just to avoid being unfairly excluded from decentralized negotiations"

Signed ACN general medicine

Rome, 29 March 2018 (press release) – The new national collective agreement for the regulation of relations with general practitioners was signed today between SISAC and the trade union organizations representing it at national level.

The agreement, in compliance with the mandate conferred by the Regions-Health Sector Committee, proceeded to define some priorities that required a particularly prompt negotiating solution pending completion of the entire NCA for the 2016-2018 three-year period. The Sector Committee paid close attention to the successful outcome of the negotiations in all negotiation phases, also responding to various needs represented by the Health Commission of the Conference of Regions and Autonomous Provinces.

These priorities pertain in particular to the need to integrate the references to the national health policy priority objectives (national chronicity plan, national vaccination prevention plan, improper access to the emergency room, management of waiting lists and appropriateness), to address the problem of generational turnover also through an adaptation of the procedures for access to positions, to innovate the regulation of the right to strike in compliance with the reference legislation for essential public services.
