This was requested by the National Federation of Nursing Professions (FNOPI), in the wake of its document presented on the occasion of the opinions on the PNRR, developed by an Advisory Board of illustrious figures from the healthcare world, in which Silvio Garattini proposed it , president of Mario Negri in Milan – to adapt nursing skills to needs, better identify the role in the various care settings also in relation to the standards of care outcomes expected in the population.

Now the associations of those directly concerned, of patient citizens, are asking for it, who have written for this to the Minister of Health Roberto Speranza, the undersecretaries of Health and the director general of the minister's programming.

Favo, Italian Federation of Oncology Volunteer Associations, Fincopp OdV, Italian Federation of Incontinent and Pelvic Floor Dysfunctions, Faip, Federation of Italian Paratetraplegic Associations, Fish, Italian Federation for Overcoming Handicap, Aistom OdV, Italian Ostomy Patients Association and AIMAR Onlus, the Italian Association of Anus Rectal Malformations and Asbi, the Italian Spina Bifida Association, which represent over 10 million citizens (about 17% of the Italian population), have asked in their letter for the essential levels of assistance to be updated so that the prescribing possibility is extended to nurses and in some cases also to physiotherapists.

The Dpcm on the Lea says that “Without prejudice to the possibility for the regions and autonomous provinces to identify the ways in which the prescription is allowed for general practitioners, paediatricians of free choice and doctors of territorial services. The products for the prevention and treatment of pressure sores are prescribed by the doctor as part of a treatment plan of a defined duration, possibly prepared by the same doctor; the prescriber is responsible for conducting the plan” and the associations formally request that this be extended, for example, also to stoma nurses, uro-rehabilitator nurses and physiotherapists, as far as they are concerned.

"We dare to point out - reads the letter - that in England for over thirty years nurses have been able to prescribe disposable medical devices to patients and no one complains, but everyone is thankful",

This is because, they explain, "the nurse, a proximity professional, if trained ad hoc is able to guarantee the taking charge of the assisted person also with regard to the prescription of disposable medical devices, as well as to favor the de-bureaucratisation of procedures (PDTA) for their acquisition. The same goes for the important figure of the physiotherapist, especially in the field of incontinence and rehabilitation of the pelvic floor”.

The motivation doesn't stop there. The modification of the Lea according to the associations would avoid "frightening bureaucratic procedures that citizens have to undergo to obtain disposable medical devices every two months (bags, plates, catheters, tracheal cannulas, incontinence pads, underpads, etc.) by the Regions, by the local Health Authorities and by the social-health Districts, under pressure from their associates and tired of undergoing bureaucratic procedures at the limit of human dignity".

"Changing course on therapeutic interventions thanks to the expansion of skills, starting from the possibility of prescribing certain classes of drugs and aids that fall within their sphere of knowledge and competence" appears to be an inevitable path, underlines the president of FNOPI Barbara Mangiacavalli. "But also - he adds - on assistance interventions, defining the full and exclusive function of care and not of supplying the other health professions, and overcoming the fragmentation and inhomogeneity of regional models".
